Eligibility Information

Employee Eligibility
- All full-time employees are eligible first of the month following, or coinciding with, their date of hire
- Part-time employees working a minimum average of 30-hours per week over a measurement period can enroll in medical plan only

Dependent Eligibility
Coverage for eligible dependents is available in the same plans you choose for yourself.
Examples of eligible dependents include:
- Legal spouse (opposite or same-sex)
- Registered domestic partner (opposite or same-sex)
- Children up to age 26
- Children who are primarily dependent upon you for support due to a physical or mental disability
